ACCC Celebrates National Case Management Week

In celebration of National Case Management Week, occurring this year from Sunday, October 8th through Saturday, October 14th, we proudly honor our dedicated team of Case Managers who are at the forefront of supporting justice-involved individuals in our community-based supervision programs. These compassionate professionals embody wisdom and expertise, making a real difference in the lives of those they serve through the industries proven evidence-based practices.

This year’s theme, “Keeping the person at the heart of collaborative care,” perfectly encapsulates the vital role our Case Managers play in the lives of individuals participating in our community-based supervision programs. They prioritize justice-involved individuals’ well-being as they work collaboratively towards the ultimate goal of achieving measurable outcomes that lead to positive life transformations.

Our Case Managers are change agents, skilled professionals, and lifelong learners. They work tirelessly to assist our clients in navigating their court and supervision requirements, removing barriers and bridging gaps to help clients access programs and services that are essential for their rehabilitation and stabilization. They are advocates, evaluators, planners, and resource coordinators all in one!

The workflow of case management is multifaceted, encompassing nine critical levels: screening, assessing, evaluating risks, planning, implementing, following up, monitoring, evaluating outcomes, and transitioning. From screening to transitioning, they guide individuals on a journey toward rehabilitation and transformation. Their commitment to achieving quality outcomes for our clients is unwavering.
